What problem does it solve?

Synchronizes the GRACE knowledge graph with the actual codebase to eliminate drift caused by refactors, multi-agent waves, or manual edits and to avoid stale dependencies and missing module entries that break tooling and navigation.

• Targeted vs full refresh: perform narrow scans of changed modules and immediate dependency surfaces or full tree scans after broad refactors.
• Graph comparison and integrity reporting: extract MODULE_CONTRACT and MODULE_MAP, compare imports and CrossLinks against docs/knowledge-graph.xml, and produce a structured GRACE Integrity Report listing missing, orphaned, and stale entries.
• Safe remediation workflow: propose concrete fixes (add/remove/update entries or generate missing contracts) and apply changes only after explicit user approval.
• Use case: after a coordinated multi-agent execution wave, run a targeted refresh to validate cross-module changes; after a large refactor, run a full refresh to reconcile the graph.
