知的障害・精神障害のある方の支援情報は多岐にわたり、一元的な管理や関係者間での共有が困難です。このスキルは、複雑な支援情報を視覚的なエコマップとして整理・可視化することで、情報管理の負担を軽減し、支援計画の策定やケース会議での情報共有を効率化します。
Core Features & Use Cases
- 対話型/ファイルモード: Claude Desktopでの対話、またはExcelファイルからのデータ入力に対応し、柔軟な情報収集を実現します。
- 関係性の可視化: 本人、家族、医療機関、福祉サービスなど18種のノードと19種のリレーションで支援関係図を自動生成し、支援の全体像を一目で把握できます。
- 履歴・処方薬管理: 手帳更新や支援区分変更の履歴、服用中の処方薬情報も一元管理し、必要に応じて表示することで、継続的な支援をサポートします。
- ユースケース: ケース会議での情報共有、サービス等利用計画の作成、担当者交代時の引き継ぎ資料として活用でき、相談支援専門員の業務負担を大幅に軽減します。
